Benefits of Care Advocacy
Our Care Advocates focus on your loved one’s specific situation and needs.  We will conduct an in-depth assessment that is different than the opening assessment for our care giving services.  The assessment considers physical, social, emotional, cognitive, safety needs, personal preferences and financial resources to develop a plan of care.  The plan of care will focus on the family and client’s personal goals so that we are a united front in the care of your loved one.

The care plan is dynamic and will change with illnesses, changes in cognitive abilities or due to many other factors.  We will monitor the plan and suggest adjustments as needs change to ensure that your loved one remains safe, happy and well cared for.

With changes in healthcare, planning care is a confusing ordeal.  We will help you navigate the system by interacting with healthcare and other service providers to make sure our clients receive quality care and are protected from those individuals that may take advantage of our senior population.

Oftentimes families of our clients live out of town.  We provide peace of mind that your loved one is getting the attention he or she needs and help to prevent a crisis before it occurs.  However, if a crisis does occur, we will be there for guidance, management and support.

Our Care Advocates may attend medical appointments with our clients and provide feedback to the appropriate family members.  If hospitalized, we will work with hospital staff and attend discharge conferences to help develop a discharge plan focused on your loved one’s goals.

  • Resource & Connection to Support Services such as Elder Law Attorneys, Veteran’s Administration, Financial Planners, and Community Resources
  • Guidance and Referrals to Preserve Financial Resources
  • Home Safety Inspection/ Equipment needs
  • Attend Discharge Meetings
  • Medical Appointment scheduling and accompaniment
